Rehabilitation refers to the diagnosis and treatment of disabilities or limitations of mobility and function following injury or illness, or the promotion of health through preventive and health and wellness activities. Rehabilitation is provided by a diverse team of specialized caregivers including physicians, physical therapists, occupational therapists and speech language pathologists.
Eastern Rehabilitation Network's rehabilitation programs are function-oriented and include setting both short-term and long-term goals, such as decreasing pain, increasing range of motion, strength, and endurance, and returning to previous activities such as work or sports. ERN encourages patients and families to take an active role in the rehabilitation program and participate in developing a care plan designed especially for them. The majority of our patients are seen for musculoskeletal and neuromuscular diagnoses. Our patients under age 18 are seen for musculoskeletal conditions. While treatment plans are individualized and based on return to function, most patients are seen for an average of 6 to 12 visits.
An essential component to achieving the best results is good communication between the patient, therapist and physician. Physicians receive timely, accurate updates on their patients' progress via telephone, written reports and personal consultation.
ERN's programs provide the tools for patients to continue with their rehabilitation after discharge, including education, home exercise programs and instruction in the use of any special equipment.
